Bridging the Transition Framework

The Bridging the Transition Framework was originally developed as a framework for guiding occupational therapists as they supported individuals across the trajectory of unhoused to housing. The focus of this framework is on supporting the prevention of ongoing homelessness through a focus on function and participation in the activities of daily life. Our team delivers a one-day workshop in this framework with health and social care providers, researchers, and policymakers in-person and virtually. Bridging the Transition to Housing: A Homelessness Prevention Framework

This book provides an overview of the Bridging the Transition Framework and its various components. As an edited book, chapters have been contributed by a range of authors from across the globe, and from multiple disciplines, including researchers and practitioners with expertise in homelessness.  

Development of the Bridging the Transition Framework

The Bridging the Transition Framework was developed by a group of Canadian occupational therapy researchers with input from international experts in occupational therapy research and practice in homelessness. Research on the initial framework development was provided by the Ontario Society of Occupational Therapists.
